Booking an Appointment

We have listened to your feedback and we have made it easier for you to access our services.

 

Online booking

You can now view, book and cancel all appointments with a GP online. This can be via the NHS App or SystmOnline. Please see Online Services.

 

Telephone 

You can call us during opening hours to speak with reception. The phone lines are very busy take calls from our patients as well as other health care professionals wanting to speak with us. 

We advise if you're call is not urgent to try later in the day, or you can use the other methods below. 

 

 

Visit Us

You are welcome to visit us at Chapeltown Health Centre between 08:30-18:00 to speak directly to a member of reception.

 

E-consult

Our e-consult service is now live. This is an online system where you can provide us with information regarding your issue. It is useful for minor illnesses, medication requests, sick note requests or chronic diseases that you feel you may not need to see a doctor about. We aim to action all e-consults within 48 hours. 

 

Email

You can also email us if you have a non urgent query. We aim to action all emails within 5 working days. 

Appointment System

Types of Appointment we offer

We offer:

  • A limited same day service with a doctor if you are acutely unwell or another urgent issue. Requests are usually triaged by the duty doctor. These appointments will usually deal with only the immediate concern.
  • A limited same day OUT OF HOURS service if you are acutely unwell you may be offered an appointment on the same day with a doctor working at a different GP surgery out of our usual opening hours.
  • Routine prebookable appointments with a doctor 
  • Routine pre-bookable appointments with a nurse 
  • Routine appointments with a health care assistant
  • Appointments with physios, pharmacists, patient ambassadors and mental health workers

Health care professionals available

We can offer direct appointments with various health care specialists who are trained to deal with certain issues. This may be a quicker way for you to get the help you need. The reception team will offer this to you if appropriate. 

  • GP - You are registered with a specific GP but you may see any GP at the practice. 
  • Extended access out of hours GP - we can offer some emergency appointments outside of our opening hours.
  • Practice nurse - for routine reviews, health checks, smears, injections, wound dressings vaccinations and other procedures.
  • Health care assistant - for a blood test, blood pressure measurement or other test.
  • Pharmacist - to discuss and review your medications. Pharmacists can prescribe if needed.
  • Physiotherapist - for joint/muscle pains.
  • Mental Wellbeing co-ordinator - to help with your mental health.
  • Patient Ambassador - if you are having a problem with housing, finances, benefits, employment or other similar issues.

Extended Practice Opening Hours

  • Extended Access GPs

This is a telephone or face to face appointment with a GP in a surgery in the local area. This will not be your usual GP.

  •  Virtual Clinical Pharmacist Service

This is a telephone appointment for patients who: need to speak to a pharmacist regarding a medication issue; but don’t need to come to the surgery. The pharmacists are able to prescribe and organise prescriptions electronically. 

  •  Virtual Musculoskeletal (MSK) Service

This is a telephone appointment for patients who need to speak to an advanced physiotherapist regarding muscle and joint problems. They will issue advice and where appropriate arrange a follow up.

Home Visits

If you need a home visit, you can help us by calling reception before 11:00.

  • You may only request a home visit if you are housebound or are too ill to visit the practice.
  • Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed. 
  • You can also be visited at home by a community nurse if you are referred by your GP.
  • You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.

Cancel an Appointment

We have made it easy to cancel your appointments online. It is important that you inform the reception staff if you are unable to attend your appointment, this will allow that appointment to be offered to another patient.

If you fail to notify the Practice that you are unable to attend, you may be sent a letter informing you that you have defaulted from your appointment. Persistent defaulters maybe removed from the list.

You can also complete our appointment cancellation notification request form. This can only be used if your appointment has been arranged for more than 24 hours in advance. (excluding weekends and public holidays).
